I am 34 years old with four children. I want to have another child, but my tubes were clamped after an ectopic fifth pregnancy. How can I begin preparing myself for reversal surgery? What can I do to increase my changes of becoming pregnant after the surgery?

The best preparation for pregnancy after bilateral tubal ligation for ectopic is to appreciate that tubal reversal is not what you need. The fact that you had an ectopic already suggests you have tubal disease in one tube, and most likely in the other. If you are certain you want to have more children, the treatment option you should be entertaining is in vitro fertilization (IVF). In terms of preparation for any pregnancy, you are best served to eat sensibly, discontinue caffeine and alcohol, exercise and keep your BMI under 25.

In general, you can try to exercise, lose weight, and prepare yourself to be as healthy as possible for surgery and pregnancy. Also, taking a multivitamin and stopping smoking and heavy alcohol consumption will also help.

In addition, stop smoking if you smoke and start a prenatal vitamin. Don't wait too long since you are 34. One tip is to ask your surgeon to check anti-chlamydia antibodies in you to make sure that the ectopic pregnancy did not occur from tubal damage due to an undetected infection. Also, have your partner's sperm count checked at a reputable fertility clinic.
