The best place to start would be with an appointment with a reproductive endocrinologist. Because not all tubal ligations can be reversed, your RE will start by reviewing the report from your tubal ligation. If your tubal is a type that can be reversed, she will then check your ovarian reserve and your partner's semen analysis. If these are normal, she can discuss the pros and cons of a tubal reversal vs IVF for women of your age. Good luck!

Tubal reversals can be performed on woman of many different ages. Age alone does not dictate whether or not a woman is a good candidate to have the procedure. Before performing a tubal reversal, it is important that the physician be able to review an operative report from the initial tubal ligation. Depending on the type of tubal ligation and the amount of tube affected can determine if a patient may or may not be a candidate for a tubal reversal. If the operative report is reasonable, a physician may then order a lab test known an AMH, or antimullerian hormone. This level is a good indicator of the "egg reserve" or the chances of becoming pregnant following the tubal reversal. If the AMH is a reasonable level, the physicians and medical staff would perform a baseline ultrasound to assess the uterus and ovaries. In addition to performing preliminary testing on the female, it may also be of significance to perform a semen analysis on the female's partner to ensure there would be adequate sperm to fertilize an egg should the tubal reversal be successful. In conclusion, it is very reasonable to assume that a forty year old female may be a good candidate for a tubal reversal.

The best way is to send your tubal ligation records to a fertility center office for review. In general most women can have successful tubal ligation reversal and over time the surgery can be more successful than IVF. At our practice we are able to reverse the tubes in over 98% of the women who come to us for reversal.
