Timing of sexual intercourse can be important to achieving pregnancy. The sperm can live up to four days in a woman’s uterus and fallopian tubes, but the egg will survive for only one day once it is released from the ovary during ovulation. In other cases a man may ejaculate too quickly, prior to insertion. Still other men may not be able to reach sexual climax and ejaculate despite maintaining a normal erection during intercourse.

A Physical Examination

A physician, particularly a urologist, can usually easily remedy erection difficulties. In many cases, the psychological stress of trying to conceive with sex on demand according to the woman’s cycle is to blame. In other cases, there is a physical impairment due to such underlying conditions as diabetes or high cholesterol. Also, low testosterone may be a factor. For many, oral medication such as Viagra, Cialis, or Levitra will be effective without any known impairment to the man’s sperm. Rapid ejaculation is very common and this condition can be effectively managed with a behavioral feedback technique that both the man and the woman practice together known as sensate focus. Therapists that specialize in sexual medicine teach this technique and it helps the man reset his threshold for the ejaculatory reflex. Pills can also help this condition. Cialis has been shown to improve rapid ejaculation. Another class of medication known as seratonin reuptake inhibitors is also effective when taken daily but recent reports suggest that this medication may effect sperm quality. For men who do not obtain effective treatment, intrauterine insemination will allow the placement of sperm effectively into the uterus.

Inhibited ejaculation is a condition not well understood by medical doctors, but it is thought to be largely a psychological condition. A therapist with sexual medicine expertise can often improve this condition. In some cases, a man requires a particular type of manual stimulation to ejaculate and if he is unable to achieve intromission, he can still father a child using manually ejaculated semen with intrauterine insemination.
